Our Social Services Program includes:

Discharge Planning

We assist residents with adjusments to Nursing Home placement and we provide support and assistance to them and their families throughout their time in this facility. Social Workers work closely with other disciplines to ensure that all of the resident's emotional and concrete needs are met, and we also act as liaison between residents and family, residents and staff, family and staff as well as residents and community-based agencies. We assist with discharge planning and it is our greatest pleasure when former residents return to the community after they have completed rehab here at Park Gardens.
